Vision and hearing screenings will be conducted by screening technicians certified by the Illinois Department of Public Health (IDPH), at our school on Tuesday, March 12th and Wednesday, March 13th. The preschool children (ages 3 and above) will be screened for both vision and hearing as required by the State of Illinois. We've attached an options and permission form that we need returned to school with any corresponding documentation by March 8th. Please print, sign, and return the form to school, or you can sign it electronically and send it back to parkwest@parkwestcoop.org.
Please note the following important information:
The preschool vision screening is not acceptable to, nor is it intended to, meet the State of Illinois Eye Examination kindergarten entrance requirement (or first-time entrance into the Illinois school system.) Please check with your child’s future kindergarten program regarding this requirement.
Before a vision screening is conducted, the State of Illinois requires written notification that states: "Vision screening is not a substitute for a complete eye and vision evaluation by an eye doctor. Your child is not required to undergo this vision screening if an optometrist or ophthalmologist has completed and signed a report form indicating that an examination has been administered within the previous 12 months." The vision screening, performed in accordance with IDPH screening standards, does not take the place of a comprehensive eye and vision examination conducted by an optometrist or ophthalmologist and will not detect all existing or potential vision problems or diseases.
*In lieu of participating in this school provided vision and hearing screening program, it is acceptable to provide the following forms to the school: 1.) a completed and signed report form, indicating that an eye examination by M.D. specializing in diseases of the eye or a licensed optometrist has been administered within the previous 12 months and 2.) a completed and signed report form, indicating that the child has had an ear examination by a physician and an audiological evaluation completed by an audiologist within the previous 12 months.
